[Abstract]

Objective:The purpose of this study was to investigate the prevalence of glucose metabolism disorders(GMD) in malignancy and compare characteristics of diabetes mellitus(DM),impaired glucose tolerance(IGT) and impaired fasting glucose(IFG) in patients with different kinds or systems of malignancy.Meanwhile,we researched the importance of performing oral glucose tolerance test(OGTT) among patients with cancer and analyze the contribution of GMD to the development of malignancy.Methods:2405 patients(as the First Group) and 123 patients(Second Group) with malignancy who had been hospitalized in the first affiliated hospital of JI NAN University during January 2002 to June 2007 and August 2007 to April 2008 were treated as case groups.2016 non-cancer people who finished the health examinations in affiliated hospital of Guangzhou Medical College or hospitalized in the first affiliated hospital of JI NAN University during September 2007 to June 2008 were used as control group.We compared metabolism of glucose,lipids and uric acid in patients with different kinds or systems of malignancy.Gradated by ages,we contrast characteristics of glucose metabolism in case groups and control group.OGTT was used for the characterization of the glucose metabolism in the Second Group.Results and significances of OGTT were studied.Based on logistic regression models,we analyzed influential factors of development of carcinoma.Results:1.Among cancer patients,ratio between male and female was 1.3:1,56?70 age stage accounted for the largest part,the morbidity of lung cancer which reach 15.1%is the highest.2.In 2528 malignancy patients,prevalence of diabetes mellitus (DM) and impaired fasting glucose(IFG) reached 27.9%.The older had the higher risk to become GMD.Digestive and hematological system carcinoma,especially pancreatic cancer,intestine cancer and leukaemia showed most obviously glucose metabolism disorders,the former one with hyperglycemia but the latter two with hyperglycemia or hypoglycemia.3.The risk of hypoglycemia was raised in case group,the younger the higher.Leukemia and esophageal cancer accounted to 12.5% and 12.1%respectively were the easiest ones to become hypoglycemia.4.If diabetes patients developed malignancy,they were most likely to develop intestine cancer and lung cancer.5.In carcinoma patients the morbidity of hypercholesterolemia,hypotriglyceridemia and hypouricemia was elevated.6.Fasting plasma glucose represented a risk factor for lung cancer,breast cancer,leukaemia, lymphoma,thyroid cancer,bladder cancer and pancreatic cancer;DM raised the risk of developing intestine cancer,liver cancer,esophageal cancer,thyroid cancer, cervical cancer and pancreatic cancer.7.GMD in cancer patients probably manifested as higher postprandial plasma glucose.If only resorted to fasting plasma glucose, merely 26.0%GMD could be diagnosed.However,if we performed OGTT among cancer patients,71%GMD could be diagnosed,including 25%DM,3%IFG,38% IGT and 5%IFG+IGT.Conclusions:The morbidity of GMD and hypoglycemia is elevated among malignancy patients,especially digestive and hematological system carcinoma,and pancreatic cancer with hyperglycemia,intestine cancer,liver cancer and leukemia with hyperglycemia or hypoglycemia.Hyperglycaemia represented a risk factor for lung cancer,breast cancer,leukaemia,lymphoma,thyroid cancer,bladder cancer and pancreatic cancer;DM raised the risk of developing intestine cancer,liver cancer, esophageal cancer,thyroid cancer,cervical cancer and pancreatic cancer.GMD in cancer patients probably manifested as improved postprandial plasma glucose and performing OGTT can avoid missed diagnosis.
